Job Summary

The primary role of the Patient Care Coordinator is to support the Care Team (Doctor, Nurse, Dietician, Physiotherapist, etc) by coordinating care to patients on the provider’s daily schedule and by proactive interventions

• Will act as a conduit for the transmission of information between providers and patients.

• Will coordinate Medical and Support services for all patients.

Job Responsibilities

• Welcome, identify and address patients at the entrance in a professional manner.
• Help patients in registering/ billing / escort the Cash & TPA patients to service areas.
• Escort and assist patients requiring special attention
• Accompany patients throughout their hospital stay.
• Coordinate patient movement between different departments like radiology, lab, pharmacy, billing, discharge etc.
• Assist patients in getting further treatment and admission as advised by the treating doctor.
• Be available and accessible on mobile phone as a single point of contact for assigned patients.
• Follow up with patients and staff to ensure that all queries and processes are resolved in a timely manner.
• Collect feedback from patients & escalate report to customer care supervisor for taking corrective measures.
• Be an ambassador for the NH culture and ensure that patients receive a world class customer experience in our hospital
• Help patient for downloading NH Care Applications and increase walk-in conversions
• Collect feedback and google reviews and increase the hospital score for google reviews
• Online conversion and investigations should be higher through NH care App.
• Facilitate the care team in coordinating care for visits and for future healthcare needs and ensure that proper care has been provided to each patient as per plan.
• Handle calls from patients if needed. Resolve the reason for the call or route to the appropriate party.
• Provide an effective communication link between patient and medical staff, including relaying messages from providers, gathering information from patients for providers, etc.
• Ensure that all patients are tracked and data entered systems for follow-up and reporting.
• Perform other duties as assigned by relevant authorities.
