Job Title: Junior Consultant Doctor

Reporting To: Senior Consultant / Head of Department.
Key Responsibilities
1. Patient Assessment & Clinical Care
-
Conduct detailed history taking and clinical examination of patients.
-
Assess patients presenting to the OPD, inpatient wards, emergency services, or assigned clinical areas.
-
Identify relevant clinical problems and establish appropriate differential diagnoses.
-
Review previous medical records, investigation reports, medications, and treatment history.
-
Develop appropriate diagnostic and treatment plans within the scope of clinical expertise.
-
Monitor patients' clinical condition and response to treatment.
-
Reassess patients when there is a change in clinical status.
-
Escalate complex or deteriorating cases to the Senior Consultant or appropriate specialist.
-
Ensure appropriate continuity of care during admission, treatment, and discharge.
2. Diagnosis & Investigations
-
Order relevant laboratory, radiological, and other diagnostic investigations as clinically indicated.
-
Interpret investigation results within the scope of competence.
-
Correlate investigation findings with the patient's clinical condition.
-
Avoid unnecessary investigations and treatments.
-
Review abnormal or critical investigation results promptly.
-
Discuss significant findings with the patient and relevant clinical team.
-
Seek specialist opinion whenever required.
3. Treatment & Medical Management
-
Initiate and monitor appropriate medical treatment in accordance with clinical protocols.
-
Prescribe medications responsibly and document the indication where appropriate.
-
Monitor therapeutic response and potential adverse effects.
-
Modify treatment plans based on clinical progress, investigations, and specialist advice.
-
Provide appropriate supportive and preventive care.
-
Counsel patients regarding medication compliance, lifestyle modifications, follow-up, and warning signs.
-
Ensure appropriate documentation of treatment decisions.
4. Inpatient Responsibilities
-
Conduct regular rounds of admitted patients.
-
Review clinical progress and update treatment plans.
-
Document progress notes, clinical findings, and management decisions.
-
Coordinate investigations and consultations.
-
Communicate significant changes in patient condition to Senior Consultants.
-
Participate in multidisciplinary discussions regarding patient care.
-
Ensure appropriate discharge planning and follow-up instructions.
-
Identify patients requiring escalation of care or higher-level monitoring.
5. Outpatient Department (OPD)
-
Evaluate patients attending the OPD.
-
Obtain relevant medical history and conduct clinical examinations.
-
Develop appropriate diagnostic and treatment plans.
-
Explain diagnosis, treatment options, expected outcomes, and follow-up requirements to patients.
-
Provide appropriate referrals to other specialties when indicated.
-
Maintain accurate OPD records and clinical documentation.
-
Ensure appropriate follow-up of patients with chronic or ongoing medical conditions.
6. Emergency & On-Call Responsibilities
Depending on hospital requirements, the Junior Consultant may:

Attend patients requiring urgent medical evaluation.
-
Perform initial assessment and stabilization within the scope of training and hospital protocols.
-
Recognize clinical deterioration and initiate appropriate escalation.
-
Coordinate with emergency, critical care, and specialist teams.
-
Respond to urgent calls from inpatient units.
-
Participate in on-call and emergency duties as per the hospital roster.
-
Ensure proper handover to the incoming medical team.
7. Coordination With Senior Consultants
-
Discuss complex and high-risk cases with Senior Consultants.
-
Seek guidance for cases outside the scope of individual expertise.
-
Participate in consultant rounds.
-
Present relevant clinical information clearly and concisely.
-
Implement agreed treatment plans and monitor patient response.
-
Communicate changes in patient condition promptly.
8. Multidisciplinary Teamwork
Collaborate effectively with:

Pharmacists

Physiotherapists

Laboratory teams

The Junior Consultant should promote respectful, professional, and patient-centered communication across the healthcare team.
9. Patient & Family Communication
-
Communicate with patients and their families respectfully and professionally.
-
Explain diagnoses, investigations, treatment plans, and follow-up requirements in language appropriate to the patient's understanding.
-
Address patient and family concerns within the scope of clinical responsibility.
-
Provide appropriate health education.
-
Maintain confidentiality of patient information.
-
Escalate difficult or sensitive discussions to the Senior Consultant when appropriate.
10. Medical Documentation
Maintain accurate and timely clinical documentation, including:

Patient history
-
Clinical examination findings
-
Diagnosis and differential diagnosis

Investigation requests and results
-
Treatment plans
-
Medication orders
-
Progress notes
-
Consultation notes
-
Procedure documentation, where applicable

Discharge summaries

Follow-up instructions

Referral documentation
Ensure documentation complies with hospital policies and applicable professional and legal requirements.
11. Patient Safety & Quality
-
Follow hospital clinical protocols, infection-control practices, and patient-safety standards.
-
Participate in quality-improvement initiatives.
-
Report adverse events, medication errors, and other patient-safety concerns through appropriate hospital channels.
-
Follow appropriate procedures for informed consent and patient identification.
-
Maintain professional standards at all times.
-
Participate in clinical audits and quality-assurance activities where required.
12. Clinical Meetings & Academic Activities
-
Participate in departmental meetings.
-
Attend clinical case discussions and morbidity/mortality meetings where applicable.
-
Participate in journal clubs, seminars, CME activities, and teaching sessions.
-
Keep clinical knowledge and skills updated.
-
Review relevant clinical guidelines and evidence-based practices.
-
Support teaching and mentoring of junior medical staff, interns, or trainees where appropriate.
