Clinical Pharmacist
- To coordinate pharmacovigilance program of the hospital
- To conduct medication safety audits to anticipate, prevent and investigate medication error of all types. Analysis of medication error audits.
- To scrutinize and interpret prescriptions and supervise medication management with the overall objective of ensuring patients receive the correct medicine, in a timely manner, and that the medicine is safe and appropriate for the patient.
- To actively participate in the Hospital Pharmacy and Therapeutic Committee, presenting new drug information to the PTC and monitoring the movement of non-formulary items in coordination with Pharmacy and submit a report to the Pharmacy and Therapeutic Committee periodically and presenting new drug information to the PTC
- To coordinate with doctors and forward their recommendations to Pharmacy and Therapeutic Committee for addition or deletion of drug from formulary
- To actively participate in Hospital Antibiotic Stewardship Program, Analysis of Restricted Antibiotics and providing guidance on usage by coordinating with Infection Control department.
- To coordinate with doctors and suggest dose adjustments in cases of abnormal Pharmacokinetics and Pharmacodynamics of drugs
- To coordinate with doctors and nurses and answer to queries related to proper use, storage, stability and overdose of drug
- To ensure that prescribing is in line with formulary guidelines and prescribing policies
- To check the accuracy of prescriptions, coordinate with doctors about drug interaction & solve the issue before administration.
- To coordinate with nutritionist about food drug interaction.
- To audit storage of Narcotics, High alert medications and LASA medicines. Monitor proper disposal and documentation of left-over Narcotics
- To ensure that medicines, including Controlled Drugs, are stored appropriately on wards and departments, performing stock checks and crash cart audit when required
- To ensure that wastage of medicines is minimized by efficient prescribing, indenting, supply & return
- To monitor & analyse Adverse Drug Reactions and coordinating with PVPI, performing all documentation and record management related to ADRs.
- To involve actively in training of Doctors, Nurses and paramedical staffs pertaining to safe medication.
- To counsel patients on the safe use of medicines.
